TO SCHEDULE AN APPOINTMENT ONLINE
STEP 1: Click the green “BOOK APPOINTMENT” link below to schedule an online appointment via the Kanata Physiocare Website. Please note that Dave only offers his services at the Kanata Physiocare location.
STEP 2: Under the Service & Practitioner Menu, select “1530910 Ontario Limited Dave Hanemaayer”.
STEP 3: Choose the appropriate Laser Quit Therapy Program under the Service Menu.
Follow the prompts and fill in the necessary fields to schedule your first appointment. Dave will recommend when to book your second appointment after your first treatment (usually within two weeks). Please complete the online health history intake form before your first appointment.
PLEASE NOTE: A $50 Laser Quit Therapy Prep Guide fee will be charged to your credit card (non-refundable). Subsequently, you will be emailed a PDF version of the guide. The $50 fee is applied towards the cost of your program, which is due upon completion of your first treatment.
THE PREP GUIDE: This is integral to all Ottawa Laser Solutions Laser Quit Programs. Lazer Dave will assume you have read, at minimum, the first two pages of the Prep Guide before you arrive for your first treatment.

STEP 2: Reception will collect basic information to set up a client profile and schedule your first appointment. All the Ottawa Laser Solution Programs include two treatments. Dave will recommend when to book your second appointment after your first treatment (usually within two weeks). Please complete the online health history intake form before your first appointment.
STEP 3: Reception will request a $50 payment for the Laser Quit Therapy Prep Guide (non-refundable). This is completed by etransfer to kanata@physiocarephysiotherapy.com or credit card. Upon completion of payment, you will be emailed a PDF version of the Prep Guide.
