Payment must be completed same day when scheduling by phone via etransfer (emails for each location immediately below) or credit card. When scheduling online, our missed appointment fee policy applies instead which allows us to charge you $50 in the event you miss your first appointment.
  2. Laser Quit Therapy Prep Guide. After making your first appointment, you will be emailed a PDF version of the Laser Quit Therapy Prep Guide. The Prep Guide is integral to all Laser Quit Programs. Lazer Dave will assume you have read, at minimum, the first 2 pages and taken reasonable preparation measures before your first treatment.
  3. Second Appointment. The second appointment (included with all program fees) is scheduled at the end of the first treatment (usually within 1-2 weeks).
  4. Cancelling/Rescheduling. Please commit to your designated laser quit therapy appointment time(s). If you cancel less than one business day before any scheduled appointment, we reserve the right to charge you a $50 late cancellation fee. If we perceive a cancellation/rescheduling pattern, we reserve the right to not reschedule further appointments and to discontinue treatment at our discretion.
